    Solved it. It doesn’t work on firefox. I had to open up Microsoft Edge for it to work

    Actually, the issue is not Firefox (I tried with Chrome, FF & Edge), but most likely an ad blocker extension/addon/plugin you use in FF. I had to disable AdBlockPlus for the form to appear, which is not great, but whatever.
